Eligibility

SUBMIT

 

The Applicant has to apply in prescribed Application Form (in two copies) to Naib Tahsildar (S.G.Y.) of the concerned Tahsil Office. The following additional documents have to be submitted by the following class of beneficiaries:-

          1.    Elderly:   

          Income Certificate signed by concerned Talathi & Revenue Inspector

          Proof of Residence Proof of Age

          Two copies of photograph

          Affidavit declaring non-residence of any Govt. Financial Institute or Authority Aided Institute or home.

 

2.    Disabled:

Income Certificate signed by concerned Talathi & Revenue Inspector

Proof of Residence

Proof of Age

Two copies of family photograph

Affidavit declaring non residence of any Govt., Financial Institute or Authority Aided Institute or home Birth Certificate of Children

Husband's Death Certificate

 

          3.    Widows-

         Income Certificate signed by concerned Talathi & Revenue Inspector 

         Income Certificate signed by concerned Talathi & Revenue Inspector

         Proof of Residence

         Proof of Age  

         Two copies of family photograph

         Affidavit declaring non residence of any Govt., Financial Institute or Authority Aided Institute or home

         Birth Certificate of Children

        4.    Destitute Women:-

a.    Abandoned Women: Certificate from concerned Police Patil & Sarpanch for abandonment of women

b.    Physically harassed women: Certificate from Medical Superintendent of Urban/Rural Hospital / Medical College

                                                           Certificate from concerned P.S.I/P.I.

c.    Women in divorce process: Certificate from concerned Police Patil and Sarpanch

d.    Wives of prisoners who are family head: Certificate from Prison Officer

e.    Liberated prostitutes: Certificate from concerned P.S.I./P.I.

                                        Certificate from Social Activates of Women & Child Development Department

f.     Orphaned girls: Certificate from concerned Gramsewak/ Chief Officer, Municipal council/ Ward Officer of Municipal Corporation.

 

   5.    Family benefit: Age certificate of the deceased Death Certificate

                               Certificate issued by talathi /Gramsevak  certifying deceased was  Earning Head of the family

                               Income certificate issued by talathi /Gramsevak and countersigned by Tahsildar.

 

            6.    Rural landless labourer: Any Age proof viz. Certificate of age issued by Medical Officer

                                                     Age in electoral roll

                                                     School leaving certificate

                                                     Landless remark by Talathi on Schedule 'C'
